Thank you for your interest in Engage Health! Click on the link below to get started. We suggest using a desktop. Here is what to expect:

1. Fill out your basic information and enter your payment details to sign up with our clinic. “Add More Details” button in the enrollment page is not required for sign up. If you are signing up multiple members of your family, you will need to fill out a separate form for each person.

2. You will be billed a prorated amount for your first partial month, which covers the period from your sign-up date to the 1st of the following month (our regular billing date). This initial charge will occur on the day you sign up. After your first partial month, you will be billed on the 1st of every month.

3. Call us at 480-660-6451 to schedule your first appointment. Your first appointment can be scheduled for as soon as 2 business days after you sign up.
